Request for Reimbursement

Our form is currently under construction to support more streamlined reimbursements via Zelle.  If you need to submit a reimbursement, please reach out to treasurer@chmsptsa.org for a link to the beta version of the new form.  Thank you.

Use this form to request reimbursement for previously authorized PTA expenses (including Mini Grants and Lunch Club Grants). 

Please note you will need to attach a pdf of your receipts.  If you have multiple pdfs, you can merge them into a single file for free here.

Use this form to request payment to a third party for previously authorized PTA expenses.
